A fit note can be issued following a health and work assessment by a healthcare professional and will only give you a fit note if your health affects your fitness for work.
The fit note is intended to support you to stay in, or return to work.
You also do not need one if you are off sick for 7 calendar days or less.
Fit notes are also sometimes called doctor's notes, sick notes, medical certificates or MED3.
You can complete a request online with AccuRx , which will be passed to the doctor to review.
*If you are under the care of a hospital, your certificate will be issued by the hospital, rather than by the practice.
Many employers have their own self-certification forms. They may also use an SC2 form from HM Revenue & Customs Employee's Statement of Sickness.
